Istra¾ivaèi su ustanovili vezu izmedju pilula za kontracepciju, sa visokim nivoom estrogena, koje su mnoge ¾ene koristile decenijama, i zakreèenja arterija koje mo¾e da vodi do srèanih oboljenja. Medjutim, lekari nagla¹avaju da milioni ¾ena trenutno koriste nove pilule sa manje estrogena i da ova nova studija ne znaèi da treba da prestanu da ih uzimaju.

Vi¹e od dve i po hiljade ¾ena izmedju 35 i 55 godina uèestvovao je u studiji. Istra¾ivaèi su ¾eleli da ustanove da li pilule za kontracepciju sa veæim nivoom estrogena imaju bilo kakav uticaj na stvaranje naslaga u arterijama. U studiji nisu ispitivane pilule sa niskim nivom estrogena. Rezultat je bio poveæanje od 20 do 30 odsto naslaga u arterijama za svakih 10 godina kori¹æenja pilula. I to je sluèaj èak i kada su ¾ene prestale da ih uzimaju. Belgijski kardiolog Ernst Rajt¹il predvodio je istra¾ivanje.

«Znali smo da je postojao rizik od stvaranja naslaga za vreme kori¹æenja pilula, ali takodje je bilo jasno da jednom kada prestanete da ih uzimate, vi¹e niste izlo¾eni riziku. Medjutim, na¹i podaci ukazuju da ako uzimate pilule u du¾em vremenskom periodu, imate nagomilavanje o¹teæenja koje vas izla¾e riziku èak i kada prestanete da ih uzimate,» ka¾e doktro Rajts¹il.

Drugi kardiolozi tvrde da ova studija predstavlja  poziv na uzbunu za mnoge ¾ene i njihove lekare. D¾enifer Mieres je direktorka za nukleranu kardiologiju na Njujor¹kom univerzitetu.

«To nas podseæa da srèana oboljenja poèinju rano i veoma tiho. Veæina ¾ena i ne razmi¹lja o srèanim oboljenjima do svojih postmenopauznih godina.»

Doktorka Mieres ka¾e da ¾ene koje uzimaju pilule za kontracepciju moraju da obrate pa¾nju i na ostale faktore rizika kao ¹to su pu¹enje, visok krvni pritisak, holesterol, ishrana i ve¾banje. Istra¾ivaèi i drugi lekari pozivaju na izradu dodatnih studija o efektima pilula sa vi¹im nivoom estrogena koje se danas koriste.

Nu¾an oprez pri uzimanju kontraceptivnih pilula

Struènjaci upozoravaju na to da je reproduktivnost nacije u Srbiji ugro¾ena i da devojke treba da koriste kontraceptivne pilule kako bi izbegle ne¾eljenu trudnoæu, ali ipak treba biti pa¾ljiv u odabiru pilula i nikako ih uzimati bez saveta lekara. U Australiji je jedna studentkinja umrla zbog pilula za kontracepciju „Yasmin“, proizvoðaèa „Bayer“, koje se koriste i u Srbiji.

Dvadesetèetvorogodi¹nja Tanja Hejs pilule je uzimala na preporuku lekara kao deo terapije, uz jo¹ jedan lek, zbog dugotrajnih problema sa aknama. Tokom èetiri meseca kori¹æenja, ignorisala je simptome poput nestajanja daha i te¹kog ka¹lja, pa je odjednom i kolabirala. Pet sati kasnije umrla je zbog pluæne embolije, to jest stvaranja krvnog ugru¹ka u pluæima. Australijski lekari su konstatovali da su krvni ugru¹ci uzrokovani faktorima povezanima s uzimanjem oralne kontracepcije.

- Taèno je da je venska tromboza ne¾eljeni efekat kod kori¹æenja pilula za kontracepciju. Meðutim, tromboza ne ubija momentalno. Da je kojim sluèajem devojka po pojavi simptoma prestala da pije pilule, ni¹ta joj se ne bi desilo - obja¹njava za „Blic“ dr Ana Mitroviæ, ginekolog u GAK „Narodni front“ i predsednica Udru¾enja za reproduktivno zdravlje.
I u farmaceutskoj kompaniji „Bayer“, proizvoðaèu „Yasmin“ pilula za kontracepciju zbog kojih je devojka umrla, ka¾u da je u pitanju retko ne¾eljeno dejstvo koje se mo¾e javiti pri kori¹æenju svih kontraceptivnih pilula. Venska tromboza je kao ne¾eljeno dejstvo takoðe nagla¹ena u uputstvu za kori¹æenje kontraceptivnih pilula.
Jedna od ideja lekara, nastavlja dr Ana Mitroviæ, jeste da se pacijenti edukuju kako ni¹ta ne bi radili na svoju ruku i kako bi bili u stanju da blagovremeno prepoznaju simptome. Bol u potkolenici, grudnom delu i bilo kakav problem s disanjem razlozi su zbog kojih se odmah treba javiti lekaru.
Kontraceptivne pilule sadr¾e najèe¹æe dve vrste hormona (estrogen i progesteron) koji imaju za cilj da spreèe ovulaciju kako bi ¾enu za¹titili od ne¾eljene trudnoæe.
- Pilule su nekad sadr¾ale veæu dozu hormona, koja je sada smanjena zbog rizika. Postoji mnogo pilula i svaka je razlièita. Potrebno je sa svakom pacijentkinjom razgovarati i uvidom u medicinsku dokumentaciju odluèiti ¹ta je za nju efikasno i bezbedno - istièe dr Mitroviæ.
U Srbiji se ipak, ka¾e doktorka, pilule za kontracepciju najvi¹e koriste u terapijske svrhe. Daju se pacijentkinjama koje treba da budu podvrgnute vantelesnoj oplodnji, za regulisanje menstrualnog ciklusa ili za smanjenje bola kod menstruacije. Takoðe se koriste u terapiji i prevenciji cista na jajniku, ali i u kozmetièke svrhe kod problema sa ko¾om.

Rizik od venske tromboze
Kod ¾ena koje ne koriste kontraceptivne pilule rizik od venske tromboze je 10 na 100.000. Ukoliko koriste pilule, verovatnoæa za dobijanje venske tromboze je 20 do 40 ¾ena, dok je rizik najveæi kod trudnica i on iznosi 60 na 100.000 ¾ena.

Studentkinja iz Zadra umrla od antibebi pilula?

Studentkinja èiji su inicijali H.R. (22) iz Zadra navodno je umrla od pluæne embolije izazvane pilulama za kontracepciju "jasmin", od kojih je nedavno preminula jedna devojka iz Australije, prenose hrvatski mediji.

H.R. je prvo pala u komu, a zatim preminula. Sahranjena je bez obdukcije, na zahtev roditelja, tako da je pod znakom pitanja hoæe li se sa sigurno¹æu utvrditi da li su pilule uzrok smrti.

Farmaceutska kompanija ''Bajer'' juèe je poslala upozorenje hrvatskim apotekama da su "jasmin" tablete riziène za kori¹æenje.
U tom dopisu su informacije u vezi s hormonskim kontraceptivom "jasmin", pilulom koja je od nedavno pod pa¾njom javnosti nakon ¹to je Tanja Hejs (24) u Australiji umrla od posledica izazvanih uzimanjem "jasmina".

Yasmin prevents ovulation (the release of an egg from an ovary) and also cause changes in your cervical and uterine lining, making it harder for sperm to reach the uterus and harder for a fertilized egg to attach to the uterus.

Yasmin is used as contraception to prevent pregnancy. It is also used to treat moderate acne in women who are at least 14 years old and have started having menstrual periods, and who wish to use birth control pills to prevent pregnancy.

This medication is also used to treat the symptoms of premenstrual dysphoric disorder (PMDD), such as anxiety, depression, irritability, trouble concentrating, lack of energy, sleep or appetite changes, breast tenderness, joint or muscle pain, headache, and weight gain.

Yasmin may also be used for other purposes not listed in this medication guide.
Important information about Yasmin
This medication can cause birth defects in an unborn baby. Do not use if you are pregnant. Do not use Yasmin if you have any of the following conditions: a history of stroke or blood clot, circulation problems (especially if caused by diabetes), a heart valve disorder, breast or uterine cancer, abnormal vaginal bleeding, kidney or liver disease, an adrenal gland disorder, severe high blood pressure, migraine headaches, or a history of jaundice caused by birth control pills.
Before taking Yasmin
Do not take Yasmin if you are allergic to drospirenone or ethinyl estradiol, or if you have:

    *

      uncontrolled high blood pressure, migraine headaches, or a heart valve disorder;
    *

      a history of stroke, blood clot, or circulation problems of diabetes;
    * kidney or liver disease;
    *

      adrenal gland disorder;
    *

      unusual vaginal bleeding;
    *

      any type of breast, uterine, or hormone-dependent cancer; or
    *

      a history of jaundice caused by birth control pills.

Before using Yasmin, tell your doctor if you have any of the following conditions.

    *

      high blood pressure or heart disease;
    *

      high cholesterol or if you are overweight;
    *

      liver cancer;
    *

      gallbladder disease;
    *

      diabetes;
    *

      seizures or epilepsy; or
    *

      a history of depression, irregular menstrual cycles, or history of breast or uterine cancer.

FDA pregnancy category X. Yasmin can cause birth defects. Do not use Yasmin if you are pregnant. Tell your doctor right away if you become pregnant during treatment. The hormones in this medication can pass into breast milk and may harm a nursing baby. Yasmin may also slow breast milk production. Do not use if you are breast-feeding a baby. Drospirenone may raise potassium levels in your blood. Other medical conditions can also affect potassium levels, including liver disease, kidney disease, and adrenal gland disorders. Before using Yasmin, tell your doctor if you have any of these conditions.

How should I take Yasmin?

Take Yasmin exactly as it was prescribed for you. Do not take larger amounts, or take it for longer than recommended by your doctor. You will take your first pill on the first day of your period or on the first Sunday after your period begins (follow your doctor's instructions).

Take one pill every day, no more than 24 hours apart. When the pills run out, start a new pack the following day. Get your prescription refilled before you run out of pills completely.
You may have breakthrough bleeding. Tell your doctor if this bleeding continues or is very heavy.

If you need to have any type of medical tests or surgery, or if you will be on bed rest, you may need to stop using Yasmin for a short time. Any doctor or surgeon who treats you should know that you are using Yasmin.

Your doctor will need to see you on a regular basis while you are using Yasmin. Do not miss any appointments. Self-examine your breasts monthly to check for lumps while you are taking Yasmin.
Store Yasmin at room temperature away from moisture and heat.
What happens if I miss a dose?

Missing a pill increases your risk of becoming pregnant.

If you miss one"active" pill, take the dose as soon as you remember or take two pills at the time of your next regularly scheduled dose. You do not need to use backup birth control.

If you miss two"active" tablets in a row in week one or two, take two tablets each for the next two regularly scheduled doses (one missed tablet plus one regularly scheduled tablet for 2 days in a row). Use another form of birth control for at least 7 days following the missed tablets.

If you miss two "active" tablets in a row in week three, or if you miss three tablets in a row during any of the first 3 weeks, throw out the rest of the pack and start a new package on the same day if you are a Day 1 starter. If you are a Sunday starter, keep taking a pill every day until Sunday.

On Sunday, throw out the rest of the pack and start a new pack of pills that same day. You may not have a period that month, but this is expected. However, if you miss your period 2 months in a row, call your doctor because you might be pregnant.

If you miss one of the reminder pills in week four, skip that dose and take the next one as directed.

If you miss a pill, you may become pregnant if you have sex in the 7 days after your missed pill. You MUST use another birth control method (such as condoms or spermicides) as a back-up for those 7 days.
What happens if I overdose?
Seek emergency medical attention if you think you have used too much Yasmin. Overdose symptoms may include nausea, vomiting, or vaginal bleeding.
What should I avoid while taking Yasmin?
Do not smoke while using Yasmin, especially if you are older than 35. Smoking can increase your risk of blood clots, stroke, or heart attack caused by birth control pills.

Yasmin will not protect you from sexually transmitted diseases--including HIV and AIDS. Using a condom is the only way to protect yourself from these diseases.
Yasmin side effects
Get emergency medical help if you have any of these signs of an allergic reaction: hives; difficulty breathing; swelling of your face, lips, tongue, or throat. Stop using Yasmin and call your doctor at once if you have any of these serious side effects:

    *

      sudden numbness or weakness, confusion, pain behind the eyes, problems with vision, speech, or balance;
    *

      chest pain or heavy feeling, pain spreading to the arm or shoulder, nausea, sweating, general ill feeling;
    *

      a change in the pattern or severity of migraine headaches;
    *

      stomach pain, loss of appetite, jaundice (yellowing of the skin or eyes);
    *

      a breast lump; or
    *

      symptoms of depression (sleep problems, weakness, mood changes).

Less serious side effects may include:

    *

      breast pain, tenderness, or swelling;
    *

      freckles or darkening of facial skin, increased hair growth, or loss of scalp hair;
    *

      changes in weight or appetite, swelling of your hands or feet;
    *

      problems with contact lenses;
    *

      vaginal itching or discharge; or
    *

      changes in your menstrual periods.

This is not a complete list of side effects and others may occur. Tell your doctor about any unusual or bothersome side effect.
What other drugs will affect Yasmin?

Some drugs can make Yasmin less effective, which may result in pregnancy. Other drugs may be affected by Yasmin. Before using Yasmin, tell your doctor about all the prescription and over-the-counter medications you use. This includes vitamins, minerals, herbal products, and drugs prescribed by other doctors. Do not start using a new medication without telling your doctor.

Isti uzrok smrti konstatovan je i Tanji Hejs iz Australije, koja je tablete koristila zbog problema sa aknama.

Zvaniènih potvrda da je Helena Rukavina preminula zbog upotrebe kontraceptivnih pilula jo¹ nema, ali ginekolozi znaju da pacijentkinje koje koriste kontraceptivne pilule imaju veæe ¹anse za duboku vensku trombozu.

"Èinjenica je da upotrebom oralne kontracepcije mo¾e se poveæati rizik od duboke venske tromboze", ka¾e ginekologinja klinike "Narodni front" u Beogradu Ana Mitroviæ.

"Uop¹te u populaciji od 100.000 ¾ena, 10 ¾ena æe oboleti od duboke venske tromboze. Ukoliko koristi neki vid kontracepcije na bazi hormona, opasnost za vensku trombozu raste 30 do 40 posto", ka¾e ona.

U Srbiji su se tablete "Jasmin" pojavile pre pet godina. I u predstavni¹tvu farmaceutske kuæe Bajer-©ering ka¾u da je je njihov lek bezbedan, ¹to su potvrdile i nadle¾ne agencije.

U Agenciji za kontrolu lekova ka¾u da su tablete pro¹le sve rigozne kontrole i da nema razloga za zabrinutost graðana.

Doktorka Ana Mitroviæ upozorila je da do ne¾eljenih efekata mo¾e da doðe ukoliko se tablete ne koriste po upustvu i bez prethodno obavljenih pregleda.

"Ne smemo pilulu uzimati na svoju ruku, zato ¹to na¹a sestra, prijateljica ima doboro iskustvo sa nekim lekom. Pilulu sme da prepi¹e samo lekar koji je upoznat sa negativnim i ne¾eljenim efektima ovog leka", ka¾e ona.

U Srbiji kontraceptivnu pilulu "Jasmin" koristi 3 odsto ¾ena. Meðutim, lekari istièu da osobe koje piju kontraceptivne pilule, a osete bolove u potkolenici ili grudima ili koje ka¹lju, a uzrok ka¹ljanja nije infekcija, moraju obavezno da se jave lekaru.
